Salary: $126,464 – $187,158 per year

Job Description:
We are seeking a highly skilled and motivated Pharmacist to join our healthcare team in Plattsburgh, NY. The ideal candidate will have at least two years of experience in an inpatient hospital pharmacy setting and a strong commitment to patient care and safety. This role involves ensuring the accurate preparation, dispensing, and monitoring of medications for patients while working collaboratively with healthcare providers.

Key Responsibilities:

  • Review and interpret physician orders to ensure accuracy and appropriateness of prescribed medications.
  • Prepare, compound, and dispense medications in accordance with established standards and regulations.
  • Monitor patient medication therapies, identify potential drug interactions, and make recommendations for adjustments as needed.
  • Provide drug information and counseling to physicians, nurses, and patients.
  • Collaborate with healthcare teams to develop and implement individualized treatment plans.
  • Ensure compliance with hospital policies, state, and federal regulations related to pharmacy practice.
  • Oversee pharmacy technicians and ensure the accurate preparation of medications.
  • Maintain accurate records of medication dispensing and usage.
  • Participate in quality assurance and continuous improvement initiatives within the pharmacy department.
Qualifications:

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy program.
  • Current and active New York State Pharmacist License or eligibility to obtain one.
  • Minimum of 2 years of inpatient hospital pharmacy experience.
  • Strong knowledge of pharmacological principles, medication safety, and hospital pharmacy operations.
  • Excellent communication and interpersonal skills for collaboration with healthcare teams.
  • Ability to work in a fast-paced environment and manage multiple priorities.
