Job Description:
We are seeking an experienced and strategic Director of Case Management to lead our case management team in Rutherfordton, NC. This role is responsible for overseeing the coordination of care, ensuring efficient utilization of resources, and maintaining compliance with regulatory and accreditation standards. The Director will work closely with clinical staff and administrators to enhance patient outcomes and support organizational goals.

Key Responsibilities:

  • Provide leadership and direction to the case management team, including hiring, training, and performance evaluations.
  • Develop and implement policies, procedures, and best practices to optimize case management and discharge planning processes.
  • Oversee the assessment, planning, and coordination of care for patients to ensure continuity and quality of care.
  • Monitor resource utilization to ensure cost-effectiveness while maintaining high-quality patient outcomes.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and social workers, to address patient needs and develop care plans.
  • Ensure compliance with state, federal, and accreditation standards related to case management and discharge planning.
  • Analyze and report case management data, identifying trends and opportunities for improvement.
  • Serve as a liaison between patients, families, and healthcare providers to resolve issues and enhance satisfaction.
  • Develop and oversee the department's budget, ensuring alignment with organizational goals and financial performance.
Qualifications:

  • Active Registered Nurse (RN) license in North Carolina required; CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification preferred.
  • Bachelor’s degree in Nursing (BSN) required; Master’s degree in Nursing, Healthcare Administration, or a related field preferred.
  • Minimum of 5 years of experience in case management, with at least 2 years in a leadership role.
  • Strong knowledge of healthcare regulations, utilization review, and discharge planning processes.
  • Excellent leadership, communication, and interpersonal skills.
  • Proficiency with case management software and electronic health records (EHR).
  • Ability to manage multiple priorities and work collaboratively in a fast-paced environment.

Compensation: $102,000.00 - $145,000.00 per year
