Job Description:
We are seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to join our team in Gainesville, FL. The LCSW will provide a range of social services to patients, including assessment, counseling, and support for individuals and families facing emotional, psychological, or social challenges. This role is crucial in helping patients navigate their healthcare journey and addressing any psychosocial barriers to their well-being.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments to evaluate patients’ mental health, social needs, and support systems.
  • Develop and implement individualized treatment plans, including providing counseling and psychotherapy to patients and their families.
  • Collaborate with healthcare providers to integrate social work services into the overall treatment plan.
  • Provide crisis intervention and support for patients experiencing acute emotional or psychological distress.
  • Assist patients in accessing community resources, such as financial assistance, housing, and support groups.
  • Educate patients and families about coping strategies, mental health resources, and available support services.
  • Maintain accurate and confidential records of patient interactions, treatment plans, and progress notes in the electronic health record (EHR).
  • Ensure compliance with state and federal regulations and facility policies regarding social work practice.
Qualifications:

  • Master’s degree in Social Work (MSW) from an accredited program.
  • Current Licensed Clinical Social Worker (LCSW) license in the state of Florida.
  • Minimum of 2 years of experience in clinical social work, preferably in a healthcare or mental health setting.
  • Strong skills in assessment, counseling, and case management.
  • Excellent communication, interpersonal, and problem-solving abilities.
  • Ability to work collaboratively with a multidisciplinary team and manage a diverse caseload.
  • Proficiency in electronic health records (EHR) and clinical documentation.
